- Complete the NexGen Applicant Form and the Out-of-State Fingerprint Consent Form.
- Get fingerprinted on FD-258 fingerprint cards. Send two cards to prevent delays.
- Have the official complete the OOS-FP Identity Verification Certifying Statement (leave TCN FRM blank).
- Pay $95 + tax via Zelle, Money Order, or Payment Link.
- Include a copy of your ID (front/back) with your email + phone written clearly.
- Mail your packet (cards + forms + ID + payment proof if applicable) to NexGen Biometrics, Evergreen Park, IL.
- We scan and submit, then email your TCN number.
